The three-phase model is simple: prepare before the work starts, establish the standard on the first installation, then follow up with a running log. The breakdown is almost always in the handoffs between those three phases.

Where three-phase quality control breaks down on site
Most construction teams already know the three-phase control model: preparatory, initial and follow-up. What they don't have is a form that makes each phase a record. Without the record, the preparatory meeting gets skipped, the first install becomes the test case, and follow-ups are done 'as required' with nothing written down.

There is no preparatory meeting, so the first installation is the first time the crew thinks through the standard.

The preparatory phase report forces the submittals, materials, prior work, testing, safety hazards and acceptance criteria to be written down before work starts.

The initial inspection happens after half the work is done, so the standard is applied retroactively.

What makes a three-phase control model actually stick
Is the preparatory phase a gate or a meeting?
A meeting without a required output is optional. The test is whether work can start without the preparatory phase report being completed.
Is the first installation treated as the standard?
The initial phase should establish the workmanship benchmark on the first piece of work, not review the tenth piece after the pattern is already set.
Does the follow-up log travel with the work?
A log that lives in a truck or on a clipboard nobody copies is useless. It needs to be the record that the next inspector picks up.
Can you prove the sequence was followed?
The value of three-phase control is the chain of evidence. If the records can be edited after the fact, the sequence is just a story.
