What most often fails on a fire and smoke damper

  • Access door drywalled or ducted over by another trade, so the damper behind a rated wall can no longer be reached or tested at all.
  • Damper labels painted over or missing, so nobody can tell a fire damper from a combination fire/smoke unit or read its rating.
  • Actuator or linkage visibly disconnected but logged as 'in place' because the blades were never watched through a cycle.
  • Damper 'cycled' by hand but never dropped on the link or driven from the alarm, so a spring too weak to close under airflow passes.
  • Blades close but don't seat because duct debris or a bent blade leaves a gap, counted as closed without checking full closure.
  • Sleeve and retaining angles wrong — oversized annular space or missing angles — so the assembly wouldn't hold the barrier's rating, never checked once the wall was closed.
