- Material laid before moisture testing, so vapor drives through the slab, breaks the adhesive bond and lifts the floor in blisters weeks after handover.
- Skim coat or patch feathered over a curing compound or old adhesive that was never removed, so the patch and the new floor let go together.
- Slab flatness never checked, so every trowel ridge and low spot telegraphs through thin LVT and sheet goods once traffic polishes the surface.
- Sheet vinyl seams heat-welded or chemically welded loose or high, so they catch a mop and open into a dirt-trapping gap.
- Adhesive spread past its working time or the floor not rolled, leaving hollow spots that drum underfoot and lift at the edges.
- Cove base gapped at outside corners and lifting at the top where the wall was never clean, read as finished because the field looks good.
- A lifting seam re-glued on top of failed adhesive so it opens again, closed as repaired without lifting the material to find why it let go.
