  • Rotary airlock left bridging or leaking, so the hopper packs out and dust backs up into the clean-air side instead of discharging.
  • New filters seated without checking the tube sheet or cages, so a gap or bent cage lets dust bleed straight through the fresh filters to the clean side.
  • Interior signed off without looking for a leak path, so the pinhole or worn gasket that was passing dust downstream is reinstalled around.
