What usually sends this back
The failures are almost always the evidence, not the work
- Relief valve on the reduced side never lifted, so if the PRV fails wide open the downstream piping has no real protection.
- Bypass valve seized because it hasn't been stroked in years, leaving no way to keep water on during a valve rebuild.
